Each year, your Hydra Health and Wellness provider will perform our wellness program, which includes tests, screenings, and diagnostics that go beyond what is typically covered by insurance or Medicare. The results of this additional testing, along with one-on-one counseling with your provider, will be used to create a custom wellness plan for you.

- $20.00 any additional sick visits/month
- Routine Labs every 6 Months (CBC, CMP, HgA1c, Thyroid Panel, Lipid Panel, Vitamin D, Vitamin B12)
- Special Price on any additional lab work needed
- Preventive and Chronic Care Management
- Medication Refills (We do not write any prescription narcotics)
- 10% OFF any other services
- 24/7 Access to CRNP and/or nursing staff
- $45.00 Sick Visits for Immediate Family (lives in household)
Family (2)- $250.00 Month
- Children: 12 to 17 yrs - $50.00 per child per month (with 2 in-home family members 18 yrs and older)
